Understanding your MultiPlan benefits
MultiPlan and PHCS are provider networks, not health insurance plans. A logo on your card identifies a network your health plan may use. Your insurer or plan administrator determines benefits and processes coverage questions.
Identify the insurer or administrator on your card. Ask that organization to verify benefits, the facility’s network participation and authorization requirements. A network logo does not guarantee payment.
Treatment options to discuss
The appropriate level of care depends on an assessment. Ask your plan about coverage for the recommended services before admission:
- Medical detox
- Residential treatment
- Partial hospitalization
- Intensive outpatient treatment
- Dual diagnosis treatment
These links describe our programs; they are not a statement that your plan covers every service.
Costs, network status and approval
Ask about your remaining deductible, copayments or coinsurance, and out-of-pocket limit. Confirm whether the facility and clinicians participate in your specific network and whether prior authorization or ongoing review is required.
Verification is not a guarantee of payment. Final coverage depends on eligibility, plan terms, medical necessity and any required approvals.
